Osteomyelitis in a circumscribed non-traumatic myositis ossificans in the leg: report of a case

Abstract

Introduction: myositis ossificans is a subtype of heterotopic ossification, being a complication that occurs after musculoskeletal trauma. Its clinical presentation consists of a stony mass, with defined contours, often painless; frequently located on the extremities. Its diagnosis can be made with an x-ray, where its main differential diagnosis is a neoplasm. The first-line treatment is conservative, with the prevention of its formation being the mainstay of management. Methods: the case of a patient who was admitted under the diagnostic suspicion of osteomyelitis of the right tibia is presented; consulting for an ulcer in the middle third of the medial face, with exposure of apparent bone tissue and secondary infection. Subsequently, the diagnosis of circumscribed non-traumatic myositis ossificans in both legs was made. Results: surgical lavage was performed by the orthopedic team, obtaining cultures and isolating a sensitive germ. A targeted antibiotic treatment was established with an adequate clinical evolution; and finally, the patient was discharged with wound cleansing sessions and elastic compression measures. At the review appointment, a successful evolution of the wound was found, without bone exposure. Conclusion: it is important to highlight that the heterotopic ossifications, like any other bone tissue, can suffer the same complications when exposed to the environment.
