"RESPUESTA HEMODINAMICA Y FACILIDAD DE INTUBACION EN NIÑOS POST LIDOCAÍNA Y/O SUCCINILCOLINA" CLINICA NOEL MEDELLIN ENERO- MARZO DE 1995.

Resumen

Se realizó un estudio para evaluar la respuesta hemodinámica y la facilidad de intubación en 120 niños ASA I y ASA II sometidos a cirugía electiva en la Clínica NOEL entre enero y marzo de 1995. Se conformaron tres grupos en forma aleatoria, cada uno con cuarenta pacientes: Grupo I, recibió Lidocaína; grupo II, Succinilcolina; grupo III, Succinilcolina y Lidocaína. Se hicieron mediciones de presión arterial media y frecuencia cardíaca pre y post intubación, considerando como clínicamente significativo un aumento mayor o igual al 10%. Se hizo un análisis de t de student para proporciones en grupos independientes y se aceptó un error máximo del 2% y una confiabilidad del 95%. La facilidad de intubación se evaluó cualitativamente según la relajación de las cuerdas y se encontró que en 100% de los pacientes fue ";buena"; y adecuada para la intubación. Se encontró un aumento en la presión arterial media mayor o igual al 10% en 16 pacientes (40%) del grupo I, 26 pacientes (65%) del grupo II y en 23 pacientes (58%) del grupo III. Se observó una diferencia estadísticamente significativa ente los grupos I y II. La frecuencia cardíaca tuvo un aumento mayor o igual al 10% en 10 pacientes (25%) del grupo I, 26 pacientes (65%) del grupo II y 27 pacientes (68%) del grupo III. Se encontró una diferencia estadísticamente significativa al comparar el grupo I con el II, y el I con el III. Estos datos muestran que la Lidocaína atenúa la respuesta hemodinámica de la intubación y que la Succinilcolina aumenta significativamente la frecuencia cardíaca y la presión arterial media; este último efecto es atenuado con la aplicación concomitante de Succinilcolina más Lidocaína. ABSTRACT This study was designed to evaluate the haemodinamic response and the facility to intubate in 120 ASA I and ASA II children who underwent an elective surgery in Clínica NOEL from January to March 1995. Three groups were conformed in a randomized way, 40 children each group. Patients in Group I received Lidocaine; group II Succinilcholine and group three Lidocaine plus Succinilcholine. Mean arterial pressure and heart rate were measured pre and post intubation, and a rise greater than or equal to 10% was considered significative. A t student test for proportions in independent groups was applied, and a maximum 2% error and a reliance of 95% were accepted. The facility to intubate was evaluated qualitatively based on relaxation of vocal cords and it was found that in all the patients it was qualified as ";good"; and adequate for intubation. A rise in mean blood pressure greater than or equal to 10% was found in 16 patients from group I; in 26 patients from group II and in 23 patients from group III. A statistically significative difference between groups I and II was found. Heart rate had rise greater than or equal to 10% in 10 patients from group I, in 26 patients from group II and in 27 patients from group III. A statistically significative difference was found at comparing group I with group II, and group I with group III. This results show that Lidocaine attenuates the haemodinamic response of intubation and that Succinilcholine rises significativelly heart rate and mean blood pressure; this latter effect is attenuated with simultaneous application of Lidocaine plus Succinilcholine
