Pyoderma gangrenosum and ulcerative colitis. Case report

Abstract

Baclground: pyoderma gangrenosum is a chronic inflammatory skin disease that belongs to neutrophilic dermatoses; it is a rare disease of unknown pathogenesis. It is characterized by presenting nodules or pustules of more common appearance in the lower limbs that in a short time progresses to a painful ulceration with raised erythematous halo, of a dirty necrotic base. Around 70 % of cases are associated with other systemic disorders, being inflammatory bowel disease the most frequent. Clinical case: we describe the case of a 64-year-old female patient with erythematous nodular lesions in the armpit and glutes that progressed to painful ulcers typical of pyoderma grangrenosum, concomitantly referring bloody stool. The diagnosis of ulcerative colitis was performed by colon biopsy and pyoderma due to the presence of typical cutaneous lesions, with no histopathologic findings of other diseases. She received treatment with cyclosporine, mesalazine and prednisolone with multiple relapses, biological management was indicated achieving remission. Conclusion: this case of a patient diagnosed with pyoderma gangrenosum is presented, qith the intention to describe the evolution and typical clinical characteristics, showing representative clinical images of active lesions and their residual scar.
