Aproximación artroscópica de tenosinovitis traumática de la vaina sinovial tarsiana: reporte de caso

Abstract

Se reportó el caso de una paciente equina que fue llevada al Centro de Veterinaria y Zootecnia de la Universidad CES, remitida por práctica privada Especial•Vet, la cual presentaba efusión sinovial en el aspecto medial y lateral de la vaina tarsiana y claudicación (3 de 5) en el miembro posterior izquierdo, con un tiempo de presentación de 24 horas. Se realizó evaluación radiológica digital encontrando alteraciones en el surco tarsiano, y también una evaluación ultrasonográfica hallando áreas hipoecóicas, aumento de tamaño y disrupción fibrilar sobre el tendón flexor digital profundo; luego se procedió a realizar evaluación artroscópica en la cual se observa ruptura fibrilar en el aspecto craneal del tendón flexor digital profundo y sinovitis de la vaina sinovial tarsiana. Se realizó tratamiento con antinflamatorios de tipo no esteroideo, antibioticoterapia respectiva al procedimiento invasivo realizado; reposo en pesebrera, y posteriormente (6°semana) aplicación de ácido hialurónico y plasma rico en plaquetas a nivel de la vaina tarsiana. Se realizó tratamiento parenteral con Legend® y Adequan®. Al momento de la publicación de este artículo, la yegua no presenta efusión sinovial ni claudicación alguna, y se encuentra realizando un plan de rehabilitación controlado.

Abstract

It was reported a case of a horse that was brought to the Veterinary Center of the University CES, provided by private practice Especial•Vet, which had synovial effusion in the medial and lateral area of the tarsal sheath and moderate lameness (3 out of 5) in the left hind foot with a presentation time of 24 hours. Digital radiological assessment was performed and alterations in the tarsal groove were find, and also an ultrasound evaluation finding hypoechoic areas, growth in size and fibrillar disruption oThe deep digital flexor tendon; then an arthroscopic assessment was performed and the fibrillar rupture was observed in the craneal aspect of the deep digital flexor tendon and synovitis of the synovial tarsal sheath. Treatment was conducted with nonsteroidal anti-inflammatory medications, antibiotic therapy according to the invasive procedure; rest in the box, and later (6th week) application of hyaluronic acid and platelet-rich plasma inside the tarsal sheath.

Parenteral treatment was conducted with Legend® y Adequan®. At the time of publication of this article, the mare does not show any lameness or joint effusion, and is conducting a controlled rehabilitation plan.
