Resumen

La mastitis se define como la inflamación del seno asociada o no a infección. La mastitis lactacional es la enfermedad inflamatoria de la mama más común; sin embargo, no deben olvidarse los procesos inflamatorios no asociados a la lactancia, clínicamente similares y a veces indistinguibles, pero con una evolución crónica y recurrente, con formación de absceso subareolar. Con frecuencia se encuentra infección bacteriana polimicrobiana, fuertemente asociada al tabaquismo. El retraso en el diagnóstico puede conllevar daño en la función o la estructura mamaria y secuelas permanentes. La similitud clínica con el cáncer localmente avanzado y con el carcinoma inflamatorio de la mama hace necesario que deban ser excluidos. El tratamiento definitivo requiere drenaje y resección quirúrgica de la fistula para reducir el riesgo de recurrencia. En esta revisión se exponen los aspectos más relevantes y actualizados sobre la enfermedad con el objetivo de proporcionar elementos para su abordaje diagnóstico y terapéutico.
