Comportamiento del nivel de ansiedad y del grado de dolor en pacientes con tratamiento endodóntico

Resumen

La endodoncia, como especialidad de la Odontología, atiende personas que pueden presentar dolor y ansiedad en diferentes grados motivados por situaciones diversas. El propósito de este estudio fue evaluar la relación entre la ansiedad y el dolor en 104 pacientes, entre los 15 – 75 años, de ambos sexos, que llegaron a la Clínica CES de Sabaneta y a los consultorios particulares de los investigadores y endodoncistas, para tratamiento de endodoncia sin importar el diagnóstico pulpar, previa aceptación de participar en el estudio y firmar el consentimiento informado. Se utilizó la Escala de Ansiedad Dental de Corah (DAS) para evaluar la ansiedad, y la Escala Visual Análoga para evaluar el dolor (VAS), en tres momentos: Antes, Durante y Después de los procedimientos. Al análisis de correlación (coeficiente de Pearson) no se encontró relación entre la ansiedad y el dolor en el período antes del tratamiento endodóntico (r = 0.171). En esta investigación se observó que la escala de dolor VAS es completamente reproducible, pero no es tan simple de realizar debido a que aun es una prueba subjetiva no muy comprendida por los pacientes, encontrando variaciones de la verdadera experiencia de dolor cuando se les solicitó señalar la percepción de dolor a lo largo de la línea de marcación. Se sugiere utilizar otras mediciones de dolor con una escala más sensible, por ejemplo una escala verbal que mida la intensidad de dolor de una forma más sencilla como: mucho, poco y nada. ABSTRACT Endodontics, as a specialty of dentistry, cares for the patients who sense different degrees of pain and or anxiety as each one perceives the same with more or less intensity. The purpose of this study was to evaluate the relationship between anxiety and pain in 104 patients, between the ages of 15 and 75 years, male and female, who arrived at the CES Clinic in Sabaneta (Antioquia-Colombia) and to the private surgeries of the investigators and Endodontists, for endodontic treatment without considering the pulp diagnosis. The patients who participated in the investigation did so by means of a written consent form. The Corah Dental Anxiety Scale (DAS) was used to evaluate anxiety and the Visual Analogue Scale (VAS) for pain. Before starting the treatment every patient completed the DAS and VAS survey, the latter was also completed by the patient during and after each treatment. In an analysis of correlation, using Pearson´s coefficient, no difference was found between anxiety and pain before treatment; r=0.171. During the investigation we noted that the VAS is completely reproducible, but is not easy to use with the patients, and variations were found from the trae pain experience along the marking line. We suggest that for further studies a more sensitive scale should be used such as a verbal scale that measures the intensity of the pain in a simpler way; a lot, very little and nothing.
