INTUBACIÓN DIGITAL PARA EL MANEJO DE LA VÍA AÉREA FALLIDA EN EL SERVICIO DE URGENCIAS(Digital intubation for the management of the failed airway in the emergency department)

Resumen

El establecimiento de una vía aérea definitiva constituye un paso prioritario en el soporte vital dirigido del paciente severamente traumatizado o  críticamente enfermo, y es además una habilidad propia del especialista en medicina de urgencias. Las diferentes estrategias usadas para la predicción de una vía aérea difícil casi nunca pueden ser aplicadas en el contexto del paciente emergente, por lo que el especialista en urgencias se ve enfrentado a pacientes con mayor probabilidad de tener una vía aérea con difícil intubación. Si bien en nuestros hospitales conocemos los diferentesdispositivos desarrollados para el rescate de vía aérea fallida, éstos rara vez están disponibles en los servicios de urgencias; faltan simplemente por falta de recursos para su adquisición o por disposiciones administrativas. La intubación digital es reconocida como el primer método que se usó para realizar intubación orotraqueal, pero también ha sido usada como técnica de rescate para una vía aérea fallida y en la escena prehospitalaria. Es por esto que se propone que la intubación digital sea tenida en cuenta nuevamente como técnica de rescate para ser aplicada en nuestros servicios de urgencias cuando otros dispositivos no estén disponibles para tal fin, así como por los equipos de cuidado extra hospitalario. 

 

Abstract:

 

The establishment of a definitive airway is a mainstay step in directed life support for the seriously injured or critically ill patient, and is also a defining skill for an emergency medicine-specialist. The different strategies used for the prediction of a difficult airway, can rarely be applied in the context of the emergency room patient, that is why the emergency medicine specialist faces patients with greater likelihood of having an airway with a difficult or failed intubation. In our hospitals, although we are aware of the different devices developed for the rescue of failed airways, they are rarely available in the emergency rooms. They are missing due to lack of financial resources or administrative dispositions. Digital Intubation is recognized as the first method used to perform a tracheal intubation, but it has also been used as a rescue technique for a failed airway and at the pre-hospital arena. This is why it is propose digital intubation to be considered as rescue technique to be applied in our emergency services when other devices are not available for this purpose and by the out-of-hospital care teams.
